Psycho-educational Evaluation Information

Overview: We offer Psycho-educational evaluations for children and young adults from ages 3 through 21. Below is a list of our services and fees. Evaluations are conducted by Dr. Catherine Oxner and Dr. Patricia Pruitt.

EXPLANATION OF FEES

Intake Appointment:

Prior to the evaluation appointment, the parent(s) will meet with psychologist to review child’s medical/ social/developmental/educational history. Family background information will be obtained. A full description of the presenting concern will be obtained. Parent(s) should plan on bringing to the appointment any pertinent records such as report cards, previous psycho-educational reports, IEPs, 504 Plans, etc. The fee is $120 per hour. An intake appointment typically lasts 1 to 2 hours.

Psycho-educational Evaluation:

This type of evaluation includes measures of cognitive ability and academic achievement in order to determine processing strengths and weaknesses as well as learning disabilities such as dyslexia. Evaluations are individually tailored to address the presenting concerns. For this reason, additional measures, as needed, may be used such as those used to identify behavioral/emotional difficulties, visual-motor integration functioning, and adaptive functioning. The $1000 fee for this evaluation includes one or more appointments to complete all of the testing, a thorough review of all intake and background information, test synthesis and analysis, and a written report which includes all of the above information as well as diagnoses and recommendations.

College Accommodations Evaluation:

This evaluation will be designed to fulfill the student’s chosen college/university requirements for identification as a student with a disability. A thorough review of the students IEP’s or 504 Plans while in high school will be required in order to document previous disabilities as well as accommodations that were necessary. This evaluation will include measures of cognitive ability and academic achievement as well as any other measures that may pertain to the student’s current disability. The $1200 fee for this evaluation includes one or more appointments to complete all of the testing, a thorough review of pertinent intake and background information, test synthesis and analysis, and a written report which includes all of the above information as well as diagnoses and recommendations for accommodations that will be necessary in college.

ADHD Evaluation:

The $840 fee for an ADHD evaluation includes a measure of cognitive ability, screening measures of reading and math, and behavioral checklists completed by the parent, teacher and child (if appropriate). A written report includes a thorough review of all intake and background information, test synthesis and analysis, and a written report which includes all of the above information as well as diagnoses and recommendations.

Feedback Appointment:

Following the evaluation, the parent(s) and student (as appropriate) will schedule a time to meet with the psychologist to review all test results, diagnoses, and recommendations. A copy of the written evaluation will be provided to the parent(s) during the feedback appointment. The fee is $120 per hour. A feedback appointment typically lasts 1 to 2 hours.

Face to Face Meetings with School Personnel:

At the parent’s request, the psychologist can provide consultation to the child’s school or teacher that may include information on the child’s learning strengths/weaknesses, information regarding the child’s diagnosis, and recommended academic or behavioral interventions. If requested by the parent, the psychologist may also attend IEP or 504 Plan meetings. The fee for this service is $120 per hour.

Educational Consultation:

This service includes reviewing files, IEPS, or any other research done on behalf of the student. The fee for this service is $120 per hour.

School Observation:

At the parent’s request, the psychologist can do school observations that include observation in the classroom and any other areas of the school which present difficulty for the child. The fee for school observation is $120 per hour.

Counseling:

Brief counseling is available as it relates to questions/concerns regarding a child’s diagnosis and appropriate interventions. The fee for this service is $120 per hour.

Payment: we accept cash, check, or debit/credit car payments. We do not currently accept insurance payments for our evaluation services. Payment for each service is due in full at the time of service.

Cancellation Policy

We ask that you give at least 24 hours notice if you need to cancel a scheduled evaluation session. Due to high demand for appointments, we cannot guarantee that cancellations or no-shows can be rescheduled within your desired time frame.
